Fees for Service

Out of pocket and out of network fees range from $125-$250 per session.

       *Rates vary based on length of session (50-90min) and individual or couple/family framework.  Visit Services & Scheduling for more specifics.

• Health Partners of MN

• MHS Health WI

• the Optum network (United Healthcare, UMR, Medica)

• Security Health Plan

• VACCN (VA Community Care Network)

• WI Medicaid - Forward Health (select HMOs only - call to confirm).

 

Please contact your specific insurance provider to confirm behavioral health benefits.

       * Ask about Keystone Counseling/Mary Telisak’s in/out-of-network status and your deductible, copay, co-insurance/cost share, etc.

Payment

I accept all major credit cards, cash, and check.

       * Insurance & payment arrangements are initiated upon scheduling through my secure online patient portal.

Cancellation Policy

If you are unable to attend your appointment a 24-hour notice is appreciated.  A call/text or email is appropriate.

      * A $50 cancellation fee may be applied to your account if you choose to cancel less than 24-hours in advance of a missed appointment.
